Comments: The two papers looked good, however one was incomplete and the
other had a few errors.  [from review:]  
 Made a mistake in not normalizing u2.  Also don't have to use
Gram-Schmidt process because v2 and v3 are orthonormal to v1 but not to
each other.  Just have to use orthonormal process on v2 or v3.
