1. Printed name (with preferred name in parentheses if
different from your first name)
2. Signature
3. Course number AND section number (Math 142-511 or Math
142-513)
4. Current major and career goals
5. Hometown
6. Phone number
7. Email address you use most frequently (see NOTE below)
8. Are you right-handed or left-handed?
9. Something interesting about yourself (my favorite part to read!)
10. Either on the front or back of the notecard, attach a photograph of
yourself. If you do not have a color photograph that you are willing to
sacrifice (or scan), you may make a copy of the picture on your student
ID.
This assignment will count as a homework grade. To receive full credit (10 points), you must comply with each of the following:
Bring your completed notecard to my office (Blocker 636) by Wednesday, January 28 or to class by Thursday, January 29. I would really like to meet each of you, but if I am not in my office, you may slip the notecard under my door, or you can turn it in to me during lecture. If you come to my office during office hours before the due date to introduce yourself and turn in the notecard, you will receive an additional 2 points (extra credit!).
NOTE ABOUT EMAIL: If you have your neo email account set up to forward to Hotmail, Yahoo, AOL, etc., beware of the following: When I send mass emails to all students in my Math 166 classes, you will receive a notice in your forwarding account that you have received class roster mail in your NEO account. You will then have to check your NEO email account to see the email I sent. If you then email me from your NEO account and I reply individually to your email, my response will get forwarded to your Hotmail, Yahoo, etc. account. You will NOT receive a notice in NEO saying that I have responded to your email.
